PIAA Sports Physicals and CIPPE Form
Students registering for athletics must have a current FULL SPORTS PHYSICAL examination completed and PIAA CIPPE form on file in the Registration Portal before the student-athlete is authorized to participate in practices, scrimmages or games for the upcoming school year season. Full physicals must be completed and dated on or after May 1, 2024. Re-Cert form must be completed no earlier than 6 weeks prior to the first practice of the season. To ensure clearance for participation, full physicals and re-cert forms must be submitted through the activity registration portal no later than the deadline for each season. Hard copies or emailed forms will not be accepted.
Registration Deadlines:
Fall Season - Monday, August 5th, 2024
Winter Season - Wednesday, November 6th, 2024
Spring Season - Friday, February, 21st, 2025
FULL PHYSICAL CIPPE forms are valid for 12 months from the date of the last certification (dated on or after May 1, 2024) and are required to participate in a sport during the school year. Subsequent sports, in the same school year, require the completion of a RE-CERT PHYSICAL CIPPE form to participate. For winter sports, RE-CERT forms cannot be completed prior to October 4, 2024. For spring sports, RE-CERT forms cannot be completed prior to January 20, 2025.
